To place an order by mail or fax simply download and complete the sales form in .pdf format and fax or mail it in with payment.  You can pay by credit card or personal check /money order.  Please copy the description and price of the item from our website and place it in the form.  Please do not forget to fill in your billing phone number so that we can reach you if we have questions.
